If you are able to, please print out the following forms, complete them, and bring them with you to your first appointment. If you are not able to do this, I will have the forms ready for you complete during your first session. Please let me know if you have any questions about any of the forms.

  • Located in the Panera Building, at the corner of W. Broad Street and Pennsylvania Ave.  

Parking
  • There is limited street parking available.  Plenty of parking is available in the underground parking garage.
  • The entrance to the underground parking garage is located at the end of the courtyard.  
  • You can turn into the courtyard of the Panera Building from W. Broad Street, or enter the back side of the courtyard from Pennsylvania Ave., just behind the Panera Building.
  • The parking garage has 3 levels.  Once parked, you may take the elevator marked "RETAIL ELEVATOR," which will take you up to the courtyard. 
Entering the Building
  • Walk across the courtyard toward the Panera Building.  The doors to the lobby are located between the Panera and the Koi Koi Sushi restaurants.  From the lobby, you can access the 3rd floor by taking the elevator, or the stairs.
  • Suite 321 is located on the 3rd floor at one end of the hallway.
